About the Role:
The Integrated Health Manager plays a pivotal role in coordinating and overseeing comprehensive health programs that integrate physical, mental, and social health services to improve patient outcomes. This position is responsible for developing strategic initiatives that align with organizational goals while ensuring compliance with healthcare regulations and standards. The role requires collaboration with multidisciplinary teams, including healthcare providers, administrators, and community partners, to design and implement effective health interventions. The Integrated Health Manager will analyze program data to evaluate effectiveness and identify opportunities for continuous improvement. Ultimately, this position aims to enhance the quality, accessibility, and efficiency of integrated health services within the organization and the broader community.
Minimum Qualifications:
- Bachelor’s degree in Public Health, Healthcare Administration, Nursing, Social Work, or a related field.
- Minimum of 5 years of experience in healthcare program management or integrated health services.
- Strong knowledge of healthcare regulations, compliance standards, and integrated care models.
- Proven ability to lead multidisciplinary teams and manage complex projects.
- Excellent communication, organizational, and analytical skills.
Preferred Qualifications:
- Master’s degree in Public Health, Healthcare Administration, or a related discipline.
- Experience working within community health settings or with vulnerable populations.
- Familiarity with electronic health records (EHR) systems and health data analytics tools.
- Certification in project management (e.g., PMP) or healthcare quality (e.g., CPHQ).
- Demonstrated success in grant writing and securing funding for health programs.
Responsibilities:
- Develop, implement, and manage integrated health programs that address physical, mental, and social health needs.
- Coordinate multidisciplinary teams to ensure seamless delivery of comprehensive health services.
- Monitor program performance through data collection and analysis to assess outcomes and inform improvements.
- Ensure compliance with federal, state, and local healthcare regulations and organizational policies.
- Engage with community stakeholders and partners to foster collaboration and resource sharing.
- Prepare detailed reports and presentations for senior leadership and funding agencies.
- Manage budgets and allocate resources effectively to support program goals.
- Provide training and support to staff on integrated health practices and protocols.
