We're fixing home healthcare, and we're building our presence in Detroit
Adaptive is building an AI-native home healthcare platform that enables clinicians to focus on delivering best-in-class care for their patients by leveraging software and innovative support solutions. We’re building our team in Detroit with plans to quickly develop an exceptional reputation of delivering excellent patient care.
Spend more time caring for patients—not charting.
At Adaptive Home Health, we're excited to offer our clinicians an innovative AI-native home healthcare platform designed to simplify your day. Enjoy customized schedules, optimized routing for greater efficiency, and ambient AI scribing that automatically documents your visits while you focus on your patients.
Prefer not to record during a patient visit? No problem. Our platform also includes an after-visit voice documentation option, allowing you to capture your notes afterward and leverage AI to draft your charts.
By streamlining documentation and reducing after-hours charting, our technology helps you spend less time on paperwork and more time doing what you do best, providing exceptional patient care.
The mission of this role
Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.
Objectives
Carry a full, high-quality caseload: ramp to ~30 productivity points/week at target visit quality, with OASIS accuracy that holds up to review.
Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.
Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.
Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.
What you'll own
Comprehensive in-home assessments and skilled documentation (OASIS, SOC/recert/ROC, discharge).
Direct, high-acuity patient care, with a focus on wound care
Building and managing the plan of care with physicians; supervising LPNs and HHAs on your cases.
Being the face of Adaptive to patients, families, and the referral sources who feed your pod.
Who you are / how you operate
Dedicated clinician: home health experience strongly preferred
High ownership: you treat your caseload like your own panel and your pod like your own business.
Open mindset: Energized by the opportunity to shape the playbook and refine SOPs
Tech-forward: You want to leverage software to accelerate the charting process, and you'll give us blunt feedback to make it better
Trust-builder: Patients, families, and referral partners believe you because you do what you say.
Requirements
Active Michigan RN license
Reliable transportation, valid driver's license, auto insurance
Home health / OASIS experience strongly preferred
Our guarantee
Full-time target: ~30 productivity points/week; clinicians earn more for exceeding it.
The rest of the package
$1k bonus for referrals (we’re growing)!
Benefits for full-time (medical/dental/vision, PTO)

Company at a glance
Adaptive Home Health's is a Medicare-certified home health agency with a proven track record of delivering compassionate, high-acuity care — and we're expanding our team across the United States. Founded and led by veterans, nurses, and operators who are deeply committed to the patient above all else, Adaptive has built a 300+ person clinical team trusted by major hospitals, orthopedic practices, wound centers, and cancer centers to deliver specialized skilled nursing, physical therapy, occupational therapy, wound and infusion care, and orthopedic rehab directly in the home.
Backed by a robust tech stack — built by top AI engineers and operators from Palantir, Character AI, and Citadel — we are pairing Adaptive's world-class clinical infrastructure with an AI operating system that automates administrative workflows, eliminates waste, and empowers clinicians to focus on what they do best: caring for patients.
The problem we're solving is urgent: 38% of referred home health patients never receive care, and 30% of all healthcare spend is lost to administrative burden — but every $1 invested in home health saves $2 from the broader system. We are fixing that, and we're hiring mission-driven clinicians, technologists, and operators all across the country to help us do it. If you want to work at the intersection of compassionate care and cutting-edge AI, and you want your work to genuinely matter, come build with us.
