Position Summary
The Home Health Branch Director is responsible for the overall leadership, performance, and regulatory compliance of the assigned branch. This role oversees daily operations, clinical coordination, staffing, and patient flow to ensure safe, high-quality care, strong financial performance, and continuous survey readiness.
Key Responsibilities
Branch Operations & Workflow
Leads daily branch operations to ensure efficient scheduling, timely admissions, and appropriate utilization of staff and resources.
Ensures operational processes align with company policies, clinical standards, and payer requirements.
Regulatory Compliance & Survey Readiness
Maintains continuous compliance with CMS Conditions of Participation, ACHC standards, and state regulations.
Ensures documentation accuracy, audit readiness, and timely corrective actions for identified risks or deficiencies.
Performance & Financial Management
Monitors branch KPIs including census, productivity, visit utilization, timeliness of care, revenue, and cost controls.
Develops and executes action plans to address performance gaps and drive continuous improvement.
Leadership & Staff Development
Recruits, trains, coaches, and evaluates Clinical Patient Care Coordinators (PCCs), office staff, and key branch personnel.
Builds a culture of accountability, collaboration, and patient-centered care.
Partners with HR on performance management, corrective action, and retention strategies.
Care Coordination & Interdisciplinary Collaboration
Works closely with intake, clinical leadership, scheduling, and therapy partners to ensure smooth patient transitions and continuity of care.
Supports interdisciplinary team communication to optimize patient outcomes and satisfaction.
Growth & Community Presence
Collaborates with marketing and referral sources to support census growth and community relationships.
Represents the agency professionally in the community and with healthcare partners.
Issue Resolution & Escalation Management
Addresses staffing challenges, patient concerns, and operational barriers promptly and effectively.
Escalates significant risks, compliance issues, or operational barriers to executive leadership as appropriate.
Skills & Competencies
Must haves:
Strong leadership and team development skills in a home health environment.
Experience in management or director position within home health.
Deep understanding of home health operations and regulatory requirements.
Excellent organizational, prioritization, and follow-through abilities.
Effective communicator across clinical, operational, and executive teams.
Data-driven, solution-oriented approach to performance management, strong understanding of KPIs within Home Health.
Environmental & Working Conditions
Primarily office-based with periodic travel to referral sources, and other branch locations as needed.
Fast-paced healthcare environment requiring frequent multitasking and rapid problem resolution.
Prolonged periods of sitting, computer use, and phone communication.
High level of mental focus and responsibility for regulatory and patient-care outcomes.
