The Registered Nurse will implement individual care plans and conduct regular home visits and physical assessments. They are also responsible for managing appointments, prescriptions, and maintaining accurate documentation of services provided.
Candidates must hold a current RN license and possess at least two years of experience in home health or a related field. Applicants are also required to pass a criminal background check and a tuberculosis screening test.

Work at least 10 hours a week
Ensure the individual care plans are implemented
Conduct a face-to-face evaluation with the individuals
Make weekly home visits, monthly physical assessments and training
Follow-up with appointments, prescriptions, physician orders, discharge orders, etc.
Document service provided to each individual in a timely manner
Other related duties as assigned
Current RN License
2 years of experience in home health or related field
Experience working with special needs population
Knowledge of current home management and personal care methods
