Provides clinical social work services to program participants, including crisis intervention and the development of individual treatment plans. Collaborates with directors to build and maintain relationships with community providers serving pregnant and postpartum individuals.
The role requires expertise in clinical social work and experience supporting individuals with substance use disorders. Candidates must be capable of navigating care resources and managing professional relationships within the community.
Provides clinical social work services to participants in the EMPOWERED program to support their recovery and facilitate positive behavioral changes. Responsibilities include crisis intervention, developing individual treatment plans, and assisting with care navigation by offering timely support and connections to resources. This position also collaborates closely with the Executive and Associate Directors to build and maintain relationships with community providers serving pregnant and postpartum individuals experiencing substance use disorders
