Proactively identify and close care gaps while coordinating preventative services and Annual Wellness Visits for patients. Support Nurse Practitioners with clinical tasks and manage transitions of care for patients leaving hospitals or skilled nursing facilities.
Requires a high school diploma and 1-3 years of clinical experience, with MA certification and familiarity with EHR systems preferred. Candidates should have strong organizational skills and experience with value-based care or population health metrics.

Position Summary: At Revere Health, we’re on a mission to deliver better health outcomes through proactive, patient-centered care. Our Value-Based Care (VBC) team is at the heart of that mission and we’re looking for a standout Medical Assistant who is energized by making a real difference in patients’ lives. This isn’t a traditional MA role. You’ll be part of a high-impact team focused on prevention, coordination, and closing care gaps before they become bigger problems.
