Collaborate with a team to manage and deliver patient care plans for hospice patients. Provide high-quality home-based healthcare services to geriatric and chronically ill patients in Middleburg, PA and surrounding areas.
Requires a valid RN license, an associate degree or higher in nursing, and at least two years of RN experience. Must possess a valid driver's license and proficiency with mobile technology and EMR software.

This full-time position supports patients in Middleburg, PA and surrounding areas and is eligible for $15,000 total bonus potential, including a $10,000 first year success bonus to reward the accomplishments of key achievements during your first year as well as an additional $5,000 sign-on bonus paid after 30-days of employment!

RN license in the state you work
Associate degree or higher from an accredited School of Nursing
Two years of RN experience, hospice experience preferred
Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
A commitment to consistently meet critical deadlines for charting

Demonstrated experience with tablets, mobile phones and EMR software
