Lead and collaborate within a team-based environment to manage and deliver patient care plans. Provide high-quality home health nursing services to geriatric and chronically ill patients in the designated service areas.
Must hold a valid RN license and have at least one year of RN experience. Requires a current driver's license and the ability to use EMR software and mobile devices for charting.

We are hiring a Registered Nurse (RN) for a Home Health position providing skilled nursing care to patients in their homes in the Schaumburg, Arlington Heights, Rolling Meadows and surrounding areas. This position is eligible for a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year!
