Oversees the population management of heart failure patients, ensuring optimal GDMT and coordinating discharge plans and patient education. Collaborates with quality improvement staff to maintain metrics on readmissions and length of stay while providing transitional care coaching.
Requires a Bachelor's Degree in Nursing and a valid RN license in NJ or PA, along with at least 2 years of cardiac nursing experience. Candidates must possess knowledge of heart failure disease management and a valid Class C driver's license.
The actual hiring rate will be determined based on candidate experience, skills and qualifications. This position is not eligible for an annual incentive.
Positions posted as annual salaries reflect an estimated amount for a 1.0 Full-Time Equivalent (FTE) (i.e. 40 hour equivalent)), positions for less than 1.0 FTE will be prorated.)
