The Case Manager collaborates with community partners to help veterans and their families achieve housing stability and quality of life. Key duties include conducting intakes, maintaining client records in HMIS, and coordinating essential services and resources.
A Bachelor's degree in a related field is required, with a preference for Social Work degrees and professional licensure. Candidates must possess a valid Indiana driver's license and the ability to lift up to 45 pounds.

Reports to: Team Coordinator
Classification: Regular, Full-time, Exempt, 40 hours per week
Conditions: Regular schedule Monday-Friday. Will require participation of on-call rotation.
