Provide comprehensive outpatient primary care to a primarily Medicare-aged, medically underserved patient population. Collaborate with a multidisciplinary team to manage comorbid conditions and address patients' medical and social needs.
Candidates must be licensed Nurse Practitioners or Physician Assistants with experience in primary care and current BLS/ACLS certification. Must be comfortable managing complex patient populations and traveling between local clinic centers.
