The LPN Care Coordinator manages and coordinates individualized care plans for patients, ensuring high-quality care through ongoing communication with providers and specialists. Responsibilities include conducting monthly care calls, monitoring patient progress, and educating patients on chronic conditions.
Candidates must be bilingual in English and Spanish with at least two years of patient-centered care and one year of skilled nursing experience. Proficiency in electronic medical records and the ability to perform nursing assessments are required.
