Key responsibilities include leadership and team oversight, patient and family coordination, case management, clinical oversight, documentation and compliance, quality improvement, interdisciplinary team meetings, education and mentorship, and regulatory readiness. This role involves extensive travel and home-based care. The qualified candidate must be prepared to travel several hours per day, covering multiple locations (1-10 home visits daily).
