Hospice Registered Nurse Case Manager โ€“ Leadership Role

We are seeking a Hospice Registered Nurse Case Manager to oversee patient care, mentor interdisciplinary teams, and ensure compliance with clinical best practices. ๐Ÿ”น Leadership & Team Oversight โ€“ Supervise, mentor, and ensure the competency of your assigned hospice care team, maintaining high standards of care.
๐Ÿ”น Patient & Family Coordination โ€“ Serve as the team leader, providing direction and oversight to interdisciplinary team members.
๐Ÿ”น Case Management โ€“ Oversee care plans, ensuring accurate evaluations, appropriate treatments, and optimal patient outcomes.
๐Ÿ”น Clinical Oversight โ€“ Monitor and review patient care, ensuring compliance with Medicare and hospice regulatory standards.
๐Ÿ”น Documentation & Compliance โ€“ Review and approve care plans within the Electronic Medical Records (EMR) system, ensuring accuracy and adherence to Medicare Conditions of Participation.
๐Ÿ”น Quality Improvement โ€“ Participate in agency quality assurance initiatives, reviewing outcome data and driving performance improvements.
๐Ÿ”น Interdisciplinary Team Meetings โ€“ Lead and coordinate team meetings with hospice clinicians and the medical director to ensure exceptional patient care.
๐Ÿ”น Education & Mentorship โ€“ Provide ongoing training, guidance, and documentation support for hospice team members, assisting with orientation and performance feedback.
๐Ÿ”น Regulatory Readiness โ€“ Assist the Director of Clinical Services (DCS) in maintaining compliance and survey readiness.
๐Ÿ”น On-Call Leadership โ€“ Participate in an on-call rotation, ensuring 24/7 oversight of patient care and agency operations.


What to Expect in This Role


๐Ÿš— Extensive Travel & Home-Based Care



  • Be prepared to travel several hours per day, covering multiple locations (1-10 home visits daily).

  • Travel areas may vary and are subject to change based on patient needs.

  • Deliver compassionate, hands-on care in diverse and unpredictable home environments.

  • Work in varied weather conditions, including rain, snow, and iceโ€”navigating stairs, different housing structures, and unique living conditions.

  • Utilize a tablet or laptop for real-time medical record documentation during each patient visit.


๐Ÿฉบ Management & Decision-Making



  • Apply clinical expertise to make sound decisions that impact patient care.

  • Solve moderately complex problems using experience, precedent, and sound judgment.

  • Collaborate effectively with interdisciplinary team members to improve patient outcomes.


๐Ÿ›  Knowledge & Skills Youโ€™ll Use Daily



  • Extensive hospice and palliative care nursing knowledge.

  • Ability to manage patient needs and interdisciplinary team functions

  • Proficiency in Microsoft Word, Excel, and EMR systems.

  • Strong interpersonal skills to communicate with patients, families, and caregivers in a compassionate and professional manner.

  • Sensitivity to patient privacy, HIPAA compliance, and ethical care practices.

  • Ability to adapt to the unique needs of each patient and family, ensuring they feel supported and valued.


Qualifications & Requirements


โœ… Education & Experience:



  • Bachelorโ€™s degree (or higher) in a health-related field

  • Minimum of 3 years of clinical RN experience (hospice experience required).

  • At least 1 year of supervisory or management experience in a hospice setting.

  • Strong knowledge of Medicare Conditions of Participation, Local Coverage Determinations, and regulatory standards (JCAHO knowledge preferred).

  • Proficiency with Outlook, Word, and Excel.


โœ… Licenses & Certifications:



  • Valid RN license in the state of practice.

  • Certified Hospice and Palliative Care Nurse (CHPN)

  • Active BLS certification (American Heart Association or Red Cross).

  • Valid driverโ€™s license and auto liability insurance for home visits.
