These include: * **Job Coach:** This on-site position involves supervising participants (clients) and transporting them to and from job sites. Weekend availability is required. The Job Coach will use OPARC vehicles, but must also maintain a personal vehicle that is registered, insured, and can pass an inspection. Part-time and Full-Time positions are available and the pay is $21.50 per hour and may be subject to a $2.00 shift differential after 6PM or Weekends. Responsibilities include providing direct job coaching and training to individuals with disabilities, delivering social and vocational skills training, promoting successful integration, and collaborating with case managers. Essential functions also include communication, customer service, advocacy, and promoting self-advocacy. Physical requirements include lifting up to 25 lbs independently and up to 50 lbs with assistance. * **Case Manager:** This on-site position requires managing a caseload of approximately 40 participants, coordinating and documenting meetings, and maintaining participant records. The Case Manager will work with participants, staff, care providers, and Service Coordinators to develop Individual Service Plans. Responsibilities include monitoring participant health and providing support. This role involves program development, implementation and team collaboration, as well as providing safety oversight and regulatory compliance. * **Community Coach:** The on-site Community Coach role involves providing person-centered services to adults with intellectual and developmental disabilities in the Explore Program. The Community Coach will facilitate activities, support participant goals, and ensure health and safety through direct care and supervision. They will also provide participant support and goal facilitation, supervision and safety measures, and communication and documentation of participant progress. Transportation and activity preparation are also key. The position requires driving participants using personal and/or Oparc vehicles and preparing supplies and equipment. The schedule is Monday through Friday, 9:00 AM – 3:30 PM (times may vary) and includes a 30-minute unpaid lunch. The position is located in Montclair, California. The Community Coach role involves Participant Support, Goal Facilitation, Supervision, Safety, Communication, Documentation, Transportation, Activity Preparation, Training, and Professional Development. The Job Coach, Case Manager, and Community Coach positions are all on-site roles. The Job Coach role involves community-based work. The Case Manager role is based on an assigned worksite.
