The RN Care Manager Float provides nursing care management in partnership with a part-time Care Manager (CM) Float. Responsibilities include conducting assessments, monitoring care plans, participating in home visits, and coordinating care transitions. The position requires a current Registered Nurse license in Wisconsin and two or more years of skilled nursing experience, ideally in gerontology/disabilities and/or home care.
