Responsibilities include skilled nursing care, medication administration, patient monitoring, collaboration, patient education, documentation, ADL assistance, and emergency response. Responsibilities include assisting clients with activities of daily living (ADLs) such as bathing, grooming, dressing, toileting, and ambulation; providing routine personal care and support services; preparing light meals; performing light housekeeping related to client care; observing and reporting changes in client condition; maintaining a safe and respectful environment; following infection control and safety policies; and completing required documentation. * **Requirements:** A valid New York State PCA Certification is required. Candidates must be listed as active and in good standing on the NYS Home Care Registry. They also need the ability to read, write, and communicate effectively in English, with additional languages considered a plus. Reliable transportation is preferred. Candidates should have a compassionate, dependable, and professional demeanor. The role requires the ability to lift, push, or pull up to 50 lbs and assist with client mobility.
