Position Summary:
Under the direct supervision and in coordination of the Social Services Manager, the Social Services Supervisor is responsible for the implementation, supervision and coordination of the Patient Navigation team. Assisting high-risk and vulnerable patient populations navigate medical and non-medical services, increasing access to care by identifying psychosocial barriers with the goal of aligning resources to improve access to healthcare and health outcomes.

Job Requirements
Education Required:
Bachelor’s Degree in Social Work or related field.
Experience Required:
At least 2 years of experience as a social worker, and at least 1 year of supervisory experience. Knowledge of community resources and their use to meet the needs of families.
Certifications/Licenses Required:
Valid California driver’s license and access to an insured vehicle.
Verbal and Written Skills Required:
Excellent oral and written communication skills. Bilingual (English/Spanish) preferred.
Equipment Used:
Office equipment, including but not limited to, computers, phones, copy machines, fax machines.
Working Conditions and Physical Requirements:
Prolonged periods of sitting and standing. Frequent walking. Driving within city. Lifting of no more than 15 lbs. Ability to remain calm and objectively evaluate problems. May be required to work outside normal working hours (only on specific occasions).
Universal Requirements:
Pre-employment requirements include I-9, physical, positive background and reference check results, complete application, new hire orientation, pre-employment PPDs.
