About this role

Position Summary:

Under the direct supervision and in coordination of the Social Services Manager, the Social Services Supervisor is responsible for the implementation, supervision and coordination of the Patient Navigation team. Assisting high-risk and vulnerable patient populations navigate medical and non-medical services, increasing access to care by identifying psychosocial barriers with the goal of aligning resources to improve access to healthcare and health outcomes.

 

Essential Functions of the Job:

  • Utilizing principles and techniques of supervision relating to training, direction, and evaluation of staff.
  • Accurately interpreting and applying relevant data to achieve desired results and focused on achieving SYHealth goals.
  • Facilitate comprehensive staff training to develop shared goals, and align staff with their role in accomplishing the organizational mission, and compliance with departmental performance standards. This includes conducting competency exams, assessments, quality assurance, and performance reviews, as applicable.
  • Analyzing problems and proposing solutions while dealing with changing priorities.
  • Participate/help manage quality improvement for Patient Navigation and Patient Assistance Program staff. Be available for consultation by staff for crisis situations and ongoing support.
  • Organize and maintain ongoing Patient Navigation and Patient Assistance Program meetings to discuss workflow, productivity, new trainings, and community resource updates. Maintain records of meeting minutes to be stored in the social services shared drive.
  • Comply with Community Information Exchange (CIE) regulations, attend monthly partner meetings, complete all CIE training and be available to train and support staff.
  • Maintain monthly rotation schedule of shared drive updates between social services staff.
  • Obtain written authorization from patients and create patient profile on the Community Information Exchange (CIE) system, as applicable.
  • Using needs assessment, identify programs and resources to assist patients based on the social determinants of health utilizing the PRAPARE Tool, as applicable.
  • Conducting a comprehensive psychosocial assessment of patient’s environmental and social needs, to determine which service(s) are required to meet participants’ needs and preferences in the community, as required.
  • Maintains a working knowledge of facility service areas including transportation, community characteristics, and geography.
  • Attend meetings, teleconferences, and trainings.
  • Recommend operational objectives and participate in prioritizing and implementing objectives for San Ysidro Health.
  • Identify and make recommendations for improving quality of patient services and cost-effectiveness of San Ysidro Health.
  • Recommend and participate in final determination of disciplinary actions and terminations, as well as the recommendation of individuals for special recognition.
  • Resolve conflicts among the professional personal.
  • Advise on operation system needs: develop, recommend and conduct special studies of operations.
  • Participate in periodic management reviews by project administration

 

Additional Duties and Responsibilities:

  • Maintains established departmental policies and procedures, objectives quality assurance programs, safely, environmental and infection control standards.
  • Enhances professional growth and development through participation in educational programs, current literature, in-service meetings and workshops.
  • Attends meeting as required and participates on community events and committees as directed.
  • Performs other related duties as assigned or requested.

Job Requirements

Education Required:

Bachelor’s Degree in Social Work or related field.

 

Experience Required:

At least 2 years of experience as a social worker, and at least 1 year of supervisory experience. Knowledge of community resources and their use to meet the needs of families.

 

Certifications/Licenses Required:

Valid California driver’s license and access to an insured vehicle.

 

Verbal and Written Skills Required:

Excellent oral and written communication skills. Bilingual (English/Spanish) preferred.

 

Equipment Used:

Office equipment, including but not limited to, computers, phones, copy machines, fax machines.

 

Working Conditions and Physical Requirements:

Prolonged periods of sitting and standing. Frequent walking. Driving within city. Lifting of no more than 15 lbs. Ability to remain calm and objectively evaluate problems. May be required to work outside normal working hours (only on specific occasions).

 

Universal Requirements:

Pre-employment requirements include I-9, physical, positive background and reference check results, complete application, new hire orientation, pre-employment PPDs.
