About this role

Position Summary:

Under the general supervision of the PACE Site Manager, the IDT Moderator ensures that decisions about medical, social, and supportive services are driven by the IDT decision-making process and not influenced by fiscal or external factors. PACE regulatory knowledge is expected  and compliance with San Diego PACE model, policies, and processes.

 

Essential Functions of the Job:

  • Meets regularly with Site Dyad Leadership and Technician Managers to review goals, trends, and performance measures for each team member and to facilitate change as needed.
  • Promotes the mission and vision of leadership in a positive and productive manner.
  • Works with the Technician Manager for each IDT discipline to coordinate the hiring, training, and evaluation of IDT members. Provides input, in conjunction with the Technical Manager, in annual performance reviews as it relates to IDT and team performance.
  • Ensures that team members understand the importance of their and others’ roles on the team and remain open to the cooperative dynamic of the team. Emphasizes teamwork and collaboration in all Interdisciplinary and cross-team interactions.
  • Coordinates orientation and in-service programs for staff to meet regulatory requirements and support of performance improvement. Ensures the provision of quality services within expected standards and financial targets. Ensures appropriate utilization of organizational resources.
  • Takes responsibility for the development, implementation, & executions of participant care. Facilitates activities related to new participants, in coordination with the IDT Team, including new participant orientation.
  • Ensures that participants are oriented to the program & have opportunities to express their opinions about the care & services they receive.
  • Coordinates all IDT Team Meetings including care planning and implementation; preparations for interdisciplinary conferences, daily meetings, and family conferences; and all related follow-ups. This includes the performance of routine care plan audits and all related follow-up with disciplines’ interventions and evaluations in participants’ care plans.
  • Participates in the continued development of the team to improve the team decision-making process, providing support to the team to troubleshoot, advise, teach, coordinate and deliver care and service to participants.
  • Partners and collaborates with the QAPI analyst in support of all regulatory activities and compliance measures. Including but not limited to the Service Delivery and Grievance resolutions, education, training of said SDAGs.
  • Serves as liaison between individual program disciplines, & community service providers. Ensures appropriate hospital & Skilled Nursing Facility (SNF) transitions are coordinated & implemented to uphold the SD PACE standard of care.
  • Assists in facilitating the LOA and contract processes to support timely placement and referrals of services.
  • Supports the PACE Site Manager and Medical Director in processing intake, disenrollment, and panel management activities.
  • Drives IDT engagement and ensures team decisions are documented.
  • May fill two separate roles on the Interdisciplinary team where the individual meets applicable state licensures requirements and is qualified to fill the two roles and able to provide appropriate care to meet the needs of participants.
  • In collaboration with the QAPI analyst, monitors and tracks grievances, complaints and dissatisfaction from participants to identify trends. Assists leadership in service recovery efforts.

 

Additional Duties and Responsibilities

  • Enhances professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops.
  • Encourages creative thinking, problem-solving, and empowerment as part of the facility management group to improve morale and teamwork.
  • Works closely with Directors and other Managers and Supervisors.
  • Performs other related duties as assigned or requested.

 

Job Requirements


Education Required:

  • Bachelor’s degree in healthcare related field. In lieu of education, work experience is acceptable. Minimum 1 year of experience of working with an Interdisciplinary Team. 

 

Experience Required:

  • Minimum 1 year of experience working with an Interdisciplinary team.
  • Minimum of 2 years of experience working with the frail elderly population.

 

Certifications Required:

  • CA Driver’s license with appropriate insurance coverage.

 

Verbal and Written Skills Required to Perform the Job:

  • Good written and oral communication skills. 
  • The verbal ability for an accurate and tactful explanation.

Technical Knowledge and Skills Required to Perform the Job:

  • Must possess the skills necessary to provide care for geriatric patients, must thrive in a team environment, must possess good organizational and supervisory skills, and ability to effectively handle difficult and unusual interpersonal situations.
  • Only acts within the scope of his or her authority to practice. Meet a standardized set of competencies for the specific position description established by SD PACE and approved by CMS before working independently.
  • Knowledge of medical terminology and working knowledge of Microsoft Windows, MS-Office.
  • Knowledge of health programs and eligibility, coding, and healthcare compliance and medical records.

 

Equipment Used:

  • Personal computer, Electronic Health Record system, or computerized referral system. Fax machine, telephone devices and voicemail.

 

Working Conditions and Physical Requirements:

  • Prolonged periods of sitting, minimum walking, and constant telephone communication. May be required to work evenings, holidays and weekends. Travel to various PACE sites may be required.

Universal Requirements:

Pre-employment requirements include I-9, physical, positive background and reference check results, complete application, new hire orientation, pre-employment PPDs. Compliance with all mandated vaccinations and all boosters is a term and condition of employment.
