JOB SUMMARY
Under the direction and supervision of the Clinical Supervisor and in collaboration with the physician, provides comprehensive nursing care utilizing the nursing process and diagnoses to primarily geriatric in the home.
This position is responsible for developing, implementing, and evaluating the patients plan of nursing care and making appropriate changes as indicated to meet the patients needs. Must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient served. Must demonstrate knowledge of the principles of growth and development and assess data reflective of the patients current status in relation to their age specific needs.
This position is responsible for assessment and admission, resumption or recertification of a patient as assigned, As needed and routine visits may also be required depending on needs of the agency. This position is also responsible for supervising the Home Health Aide and LPN and communicating with other disciplines assigned to the patient.
QualificationsJOB QUALIFICATIONS
Education: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
Licensure: Current Georgia or multi-state license as a Registered Nurse. American Heart Association BLS CPR required. Must have a valid drivers license and automobile insurance coverage in accordance with Hamilton Health Care System Driver Requirements policy.
Experience: 1 year experience as a registered nurse in a clinical care setting or public health preferred.
Skills: Nursing skills as defined in the Law Governing the Practice of Nursing in Georgia. Requires interpersonal skills and ability to learn supervisory skills. Knowledge of the professional nursing theory and practice to give and evaluate patient care. Knowledge of the organizational policies, regulations and procedures to administer patient care. Knowledge of medical equipment and instruments to administer patient care. Knowledge of common safety hazards and precautions to establish a safe work environment. Skill in applying and modifying the principles, methods and techniques of professional nursing to provide ongoing patient care. Skill in identifying problems and recommending solutions. Skill in preparing and maintaining records that meet state documentation requirements, writing reports, and responding to correspondence. Skill in developing and maintaining department performance improvement. Skill in establishing and maintaining effective working relationships with patients, medical staff, and the public. Ability to maintain quality control standards. Ability to react calmly and effectively in emergency situations. Ability to interpret, adapt and apply guidelines and procedures. Ability to communicate clearly. Ability to obtain and interpret information in terms of the patients needs that are age appropriate. Ability to select proper instrumentation/supplies appropriate for age group of patient served. Demonstrates consistent ability to perform clinical skills necessary in the care and treatment of a variety of ages in patients, particularly focusing on primarily geriatric.
Ability to work independently in patients homes without direct supervision. Ability to drive self to various locations on a daily basis.
PHYSICAL, MENTAL, ENVIRONMENTAL AND WORKING CONDITIONS
Works in patients homes in Hamilton Medical Center service area; able to work flexible schedule to meet patients needs. Prolonged standing and walking required for extensive periods, with ability to lift and move patients. Requires full range of motion. Manual and finger dexterity and eye-hand coordination required. Requires corrected vision and hearing to normal range. Requires working under stressful conditions or working irregular hours. Requires some exposure to communicable diseases or bodily fluids. Requires ability to utilize electronic medical record.
