Job Title: Physical Therapist

Department: Therapy

Reports to: Rehab Director, EDO

 

Summary:

Evaluate patients and develop physical therapy treatment programs in collaboration with the physician and provides physical therapy treatments to patients. Administer complex active and passive therapeutic exercises, therapeutic massages and various modalities. Instructs, motivates, and assists patients/caregivers in learning and improving activities of daily living. Supervises and directs the efforts of the Physical Therapy Assistant, if one is assigned.

Education & Qualification Requirements:

Bachelor's Degree, Master's Degree, or Doctorate Degree in Physical Therapy from an  accredited college or university. Current license/registration by state of hire as a Physical Therapist.

 

 

Essential Duties and Responsibilities:

  • Responsible for the establishment, implementation, and ongoing evaluation of treatment programs, plan of care, goals, and discharge plans for the patients as related to Physical Therapy needs and in conjunction with the physician. Revises the plans and goals based on changes in patient status.
  • Provides direct patient care services to ensure the effectiveness of Physical Therapy programs.
  • Applies heat, light, water, and electrically based treatments/modalities (i.e., ultrasound, infra-red lamp, muscle stimulation, traction, hot and cold packs, paraffin baths, whirlpool, diathermy, etc.) to stimulate muscle/nerve function and/or for pain management, as prescribed, with proven competency and available equipment.
  • Prioritizes, plans, treatment and travel times to provide for maximum efficiency, yet meet the needs of the patients.
  • Ensures orders are carried out in a professional manner satisfying professional ethics, patient rights, and State and Federal codes and regulations.
  • Facilitates effective communication amongst other clinicians providing care to the patient.
  • Assists in coordinating the Physical Therapy program with other appropriate health care providers/ disciplines providing services to the patient.
  • Participates in interdisciplinary patient care conferences as applicable and maintains ongoing communication with all disciplines involved in the patient Plan of Care.
  • Demonstrates a clear understanding of the Mission, Purpose, and Philosophy.

  • Organizes workload to complete responsibilities in an appropriate and timely manner.
  • Observe infection control and standard precautions.
  • Performs client/family education as appropriate.
  • Perform in-services as requested.
  • Utilizes correct coding for services provided.
  • Accepts responsibility to maintain patient loads compatible with work settings and company standards.
  • Follows established productivity requirements.
  • Maintains a courteous, respectful and informative approach in all facility interactions.
  • Demonstrates willingness to assist co-workers and/or to accept additional assignments as requested to support the department's efficiency and goals.
  • Responds to all Management/ Administration requests for information and assists in a timely and courteous manner.
  • Other duties as assigned.

 

Supervisory Responsibilities:

Directly supervises the Physical Therapy Assistants. Carries out supervisory responsibilities in accordance with our policies and applicable laws.

 

 

Physical Demands:

Able to lift up to 50 pounds, perform one-person transfer under routine and emergency situations, reposition a bedbound patient alone and able to sit, stand, bend stoop, kneel, pull, climb, reach, lift, and perform repetitive movements of the upper extremities.  May be exposed to unsanitary conditions in some facility or home settings. May be exposed to high crime areas within the service community.
