About this role

 

Position Summary: The Fellow is responsible for clinical duties (4 sessions per week) plus accompanying patient care administrative work.

Essential Duties and Responsibilities:

  • Maintains working knowledge of and adhere to the program Manual, agency handbook and Policies and Procedures
  • Commitment to providing cost conscious, fiscally responsible, timely, efficient, effective, safe, patient-centered, culturally competent, equitable, and participatory health care.
  • Attends all required trainings as per Agency Policy, Program Manual and Funder Guidelines.
  • Arrives on time for work, remains at work for whole shift, regular and predicable attendance.
  • Maintains appropriate boundaries and ethical standards with staff, clients, visitors and outside providers as detailed in Agency Policy.
  • Maintains positive and professional working relationship with co-workers, managers, clients, visitors and outside providers 
  • Strong and clear written and verbal communication skills
  • Excellent computer skills (included but not limited to Microsoft Word, Excel and Outlook)

Program Specific Essential Duties and Responsibilities: 

  • Collaborates with overall practice management, monitors special populations as part of the HIV team and acts as a consultant to other Medical Providers. 
  • Participates in Quality Improvement (QI) projects in collaboration and coordination with the HIV team.   
  • Assists in coordinating team-based QI projects. 
  • Any and all other duties as assigned.

 Qualifications and Education Requirements:

Physician, Nurse Practitioner, or Physician Assistant as their primary job description.

Prior training in primary care (MD/DO residency, NP residency, or clinical experience).

  • Familiarity with clinical practice in Patient-Centered Medical Home model of care required. 
  • Note: Fellow will be 1.0 FTE employed at FHCW with 0.4 FTE clinical.
