POSITION SUMMARY: 

The Outreach Mental Health Worker provides counseling in the home school, supporting school staff, and in the community. Responsibilities include developing and delivering individualized treatment plans collaboratively and in partnership with families. The Outreach Mental Health Worker assists the families in finding appropriate resources and provides crisis support as required. This worker must be able to work autonomously within a multi-agency team context. 

 

JOB RESPONSIBILITIES may include but are not limited to: 

  • Providing Intensive home, school and community-based assessment and treatment intervention for children & youth in Brantford, and Brant County (ages 0 – 18) identified with mental health needs, and their families. 
  • Service Coordination and collaboration in developing individualized goal-focused treatment plans. 
  • Provide consultation, support and training to families, schools and other agencies.
  • Must be able to work flexible hours to meet the needs of the families. 
  • Caregiver skill building, family sessions and group work are integral and essential components of service delivery.
  • Implement client focused activities, responsive to the needs of clients 
  • Maintain confidentiality in all aspects of the job (written/verbal/implied) 
  • Participating/Facilitating as appropriate case conferences, staff meetings, team meetings, etc 
  • Being accessible and accountable to the team members and to participate in regular supervision processes 
  • Maintaining all records and reports as required by Woodviews’s policies and procedures 

 

Qualifications:

  1. Must have Child & Youth Worker (CYW) diploma, or degree/diploma in related field
  2. Minimum 3 years’ experience in children’s mental health
  3. Demonstrated experience working with parents, caregiver, and families, including the delivery of structured and informal caregiver skill-building interventions (e.g., psychoeducation, coaching, modelling, and guided practice)
  4. Proven ability to support caregivers in building skills related to emotion regulation, behaviour management, communication, boundary setting, safety planning and crisis prevention, tailored to the developmental and mental health needs of their child or youth
  5. A demonstrated ability to work from a strength-based, collaborative perspective with clients and families with complex needs, including anxiety, depression and self-harm
  6. Ability to plan and implement client-focused activities that are responsive to individual and family needs.
  7. Sound knowledge and/or experience in the mental health field and an ability to manage the emotional and behavioural needs of clients.
  8. Experience in case management and engaging community partners in supporting children/youth and families collaboratively.
  9. Demonstrated confidence, self- direction and sound judgement.
  10. Strong ability to communicate concise, accurate information in verbal and written reporting
  11. Participation in family sessions, group work, including evening and weekend hours
  12. Maintain confidentiality in all aspects of job (written/verbal/implied)
  13. NVCI and ASIST certification an asset
  14. Current Police Check, including vulnerable screening
  15. Knowledge of DBT, CBT and other treatment modalities are an asset
  16. Valid First Aid and CPR (an asset)
  17. Strong organizational, problem solving and decision-making skills
  18. Ability to work independently as well as collaboratively within an interdisciplinary team
  19. Ability to work flexible hours
  20. Computer skills and knowledge of how to navigate different social media platforms
  21. Second language an asset
  22. Valid driver’s license and daily use of a reliable vehicle with the ability to transport clients
