Reablement - Empowering Independence
We are looking for a Social Care Practitioner to act as the main point of contact for customers and ensure they receive the right support to meet their needs. You will work closely with individuals, carrying out Care Act assessments to identify outcomes and agree how these can be achieved. This role involves coordinating care, advice, and services, including arranging equipment and support, while ensuring customers have choice and control.
You will manage your own caseload, prioritising tasks effectively and liaising with other professionals when specialist input is required. Strong communication skills are essential, as you will work with customers, carers, colleagues, and partner agencies to provide a well-organised and coordinated approach. You will also recognise the needs of carers and offer appropriate assessments and advice.
The role requires accurate record-keeping, adherence to data protection, and a commitment to safeguarding vulnerable adults. You will contribute to departmental priorities, respond to emergencies when needed, and take part in supervision and ongoing development.
We are looking for someone with relevant qualifications or equivalent experience, excellent ICT skills, and a good understanding of adult social care legislation. You should be able to work independently, manage time effectively, and maintain a person-centred approach that balances choice with risk.
