About the role:

An exciting opportunity exists for a passionate and patient-centred Grade 2 Physiotherapist to join the Western Health at Health teams working within the GEM at Home service.

 

The GEM at Home service provides patients with an alternative option to hospital-based rehabilitation by offering holistic interdisciplinary assessment and care for older people with complex, chronic or multiple health conditions within their own home environment rather than in hospital.

 

This is a fixed term part time position until June 2026. We will consider applications for up to 0.6EFT (3 days per week).

 

Key responsibilities:

The successful applicant will work in partnership with a strongly committed team to shape the provision, service development and implementation of high quality and efficient care to patients in the home environment.

 

The GEM at Home service aims to:

  • Improve the patient experience by allowing them to receive care in the comfort of their own home
  • Assist patients to achieve their care goals and best possible level of health and function
  • Promote and support safe and early discharge from inpatient settings

 

About you:

To be successful in the role, you will have:

  • Advanced clinical skills and minimum 2 years’ experience in a Physiotherapy clinical practice role, along with current registration with AHPRA and eligibility for membership of relevant professional association.
  • Experience/qualification in aged care and community-based physiotherapy.
  • Be able to independently to provide and shape a comprehensive Physiotherapy service to clients of varied diagnostic groups, whilst also working collaboratively within MDT to meet the needs of the client and service.
  • Be responsible for managing a portfolio and quality improvement projects in line with Western Health’s strategic business plan.
  • Model professional leadership, be able to prioritise tasks, work autonomously, problem solve and self-manage competing demands.
  • Highly developed interpersonal, written and electronic communication skills.
  • Excellent organisational and time management skills.
  • Commitment to continuing professional development.
  • A current Victorian driver’s licence and agreeable to undergo a police check.
