Your Role as a Senior Support Facilitator
In this role you will provide holistic non-clinical follow- up care, practical support and assertive outreach to people directly after a suicide attempt or at high risk of suicide.
The Senior Support Facilitator will work alongside the Program Manager in coordination of intake; data collection and reporting processes; providing support and guidance to the way back team; ensuring high quality standards; and upholding a positive team culture.
Key responsibilities of the role include:
Providing direction to the team on psychosocial rehabilitation, social inclusion, and recovery orientated practice frameworks.
Triaging referrals and provide consultation where necessary.
Partnering with clients and key stakeholders to conduct thorough psychosocial assessments, develop, implement and review individual recovery plans.
Providing assertive and practical support to access services related to individual needs across both outreach and in-home support.
Facilitating linkage to ongoing community supports as necessary, e.g. primary care providers, community sector agencies.
What We’re Looking For
Completed tertiary qualifications in community, social welfare, health or a related discipline.
Experience working in senior roles within a recovery orientated service framework for mental health and/or suicide prevention.
Passionate about making a positive change in the lives of people managing a suicidality.
Essential Requirements
Proof of COVID-19 vaccination status (or as required by legislation and WMQ policy)
Relevant probity checks required by legislation and WMQ Limited policy.
Current Driver’s Licence and willingness to travel for work
