This position is Full-Time/ Night Shift (7 pm - 7 am).
Job Summary:
The Care Partner Advanced (CP-A) is a support care provider who functions under the direction of the clinical nurse (Registered Professional Nurse). The CP-A primary focus will be to promote a positive patient experience by partnering with the patient, family, and other health care providers to provide excellence in customer service and assistance with daily living activities and comfort measures.
The CP-A assists other health care providers with safe, age appropriate and culturally aware care by performing basic patient care functions (i.e. ADL's, ambulation), vital signs, approved advanced technical procedures, i.e., phlebotomy, simple treatments, dressing changes, oral suctioning, and inserting and discontinuing indwelling urinary catheters and intermittent urinary catheters, discontinuing peripheral IV catheters, initiating telemetry monitoring, performing 12-lead EKG, performs insertion of indwelling/; transporting supplies/equipment/patient medications and stocking supply areas, preparing and maintaining patient charts, providing customer service and acting as a central communication source on the nursing unit and other activities as necessary.
The CP-A must be able to work in a fast-paced environment which requires multi-tasking activities and ongoing communication with the registered nurse. Must be able to perform under stressful conditions with a compassionate, respectful, and helpful manner. Must be able to take direction from a variety of care providers. Must be organized with excellent communication skills.
The Care Partner Advanced position reports directly to the Nurse Manager/Assistant Nurse Manager. Key responsibilities of the role include basic patient care functions (i.e., ADLs and ambulation), vital signs, approved advanced technical procedures (i.e. phlebotomy, simple treatments, dressing changes, oral suctioning, discontinuing indwelling urinary catheters, transporting supplies/equipment/medications.
Core Responsibilities and Essential Functions:
Patient centered care
Performs basic patient care under the direction of the Registered Professional Nurse (ADLs, ambulation)
Transport specimens, equipment, medications, blood products to and from the assigned unit
Answers call lights and telephone promptly and notifies appropriate individual of need for follow-up
Performs advanced technical procedures: phlebotomy, 12-lead EKG, dressing changes, oral suctioning, discontinuing indwelling urinary catheters and intermittent urinary catheters, initiating telemetry monitoring, discontinues peripheral IV catheters.
Teamwork
Supports initiatives of the healthcare team
Participates in unit performance improvement initiatives
Supports professional nursing practice through shared leadership, evidence-based practice and ethical accountability
Participates in orientation of staff and precepts nursing technicians on the unit
Patient safety and infection control
Provides a safe environment for the provision of patient care
Consistent use of hand hygiene
Follows appropriate PPE/Infection control policies
Performs environment of care rounds and purposeful hourly rounding
Communication
Uses positive, respectful, and effective oral and written communication with all members of healthcare team, patients, families, and visitors
Communicates patient care concerns/problems/emergencies to the RN
Uses safety communication techniques (AIDET, STAR)
Acts as liaison between patients and diagnostic areas to determine anticipated times for tests/procedures; communicates with patients and updates white boards with information
Performs other duties as assigned
Complies with all Wellstar Health System policies, standards of work, and code of conduct.
Required Minimum Education:
High School Diploma
Required Minimum License(s) and Certification(s):
All certifications are required upon hire unless otherwise stated.
Basic Life Support or BLS - Instructor
Cert Nurse Assistant-Preferred
Additional License(s) and Certification(s):
Must have a valid/current BLS card from the American Heart Association by the first day of employment. Upon Hire Required and
Certified Nursing Assistant Upon Hire Preferred
Required Minimum Experience:
Minimum 2 years clinical experience in acute care setting, long-term care facility, physician office/clinic or other direct clinical patient care experience
Basic medical terminology is preferred
Required Minimum Skills:
Ability to read, write and speak English language
