Schedule: Full-Time | Temporary (3 Months)
Pay: Starting at $40.00–$61.50/HR (DOE – based on license and experience)
Location: Eugene, OR | In-Person / On-Site

Hillside Heights Rehabilitation Center is currently seeking an experienced RN or LPN to join our team as a MDS Coordinator for 3 Months.
Position Summary
The MDS Coordinator works collaboratively with the interdisciplinary team to assess resident needs, ensure accurate MDS completion, and maintain compliance with state and federal regulations.
Key Responsibilities
Participate in the pre-admission process to gather essential information for MDS completion
Review resident care plans and assist in setting the appropriate Assessment Reference Date (ARD) for PPS MDS assessments
Ensure timely and accurate completion of resident assessments in compliance with state and federal guidelines
Accurately code MDS data using information from staff interviews, resident and family input, observations, and medical record reviews
Maintain MDS schedules and deadlines
Perform other duties as assigned
Qualifications
Current, active RN or LPN license in the state of practice
Strong interpersonal and communication skills
Knowledge of the RAI process and case management
Prior MDS experience preferred
