Are you a compassionate physician dedicated to providing exceptional care to patients and families during life’s most critical moments? About the Role:
This position primarily focuses on outpatient and home-based palliative care, with occasional inpatient consults to ensure seamless, continuous support. You’ll work alongside a collaborative team of physicians and advanced practice providers committed to compassionate, patient-centered care.

• Provide expert palliative and hospice care across home, clinic, and community settings
• Collaborate closely with a dedicated multidisciplinary team
• Participate in call and weekend rotations to support patient needs
• Offer continuity, comfort, and guidance to patients and their families

What We’re Looking For:

• Board certified or eligible in Hospice & Palliative Medicine
• Passionate about community-based, patient-focused care
• Skilled at building trusting relationships with patients, families, and interdisciplinary teams
• Motivated to grow professionally within a collaborative, innovative healthcare system

Compensation:

The starting salary for this position is: $275,000.00 (annually).
The actual compensation package could vary based on factors such as, but not limited to, the applicant’s experience, internal equity, and alignment with market data.
