Veterans Multi-Service Center (VMC) is seeking a compassionate and dedicated Permanent Supportive Housing (PSH) Case Manager to empower chronically homeless individuals and families to achieve and maintain stable housing. Key Responsibilities:

  • Implement Housing First principles to facilitate rapid housing entry and stability for participants.

  • Provide client-centered case management, building trusting relationships and supporting individualized goal setting.

  • Connect participants to vital community resources (mental health, substance use, healthcare, employment, benefits, legal aid).

  • Proactively address housing crises and advocate for participants' needs.

  • Maintain accurate client records and ensure compliance with HUD regulations and HMIS data standards.

Qualifications:

  • Bachelor's degree in Social Work, Human Services, Psychology, or related field.

  • 2-3 years of case management experience, preferably with chronically homeless individuals or HUD-funded programs.

  • Strong understanding of Housing First, trauma-informed care, and harm reduction.

  • Excellent interpersonal, communication, and organizational skills.

  • Proficiency in MS Office and HMIS/client management databases.

  • Valid driver's license and reliable transportation.

  • Empathy, resilience, and a passion for serving vulnerable populations.
