We are currently seeking a skilled & proactive Patient Flow Coordinator (RN) to join our team. This critical role ensures seamless coordination of patient placement, transfers, & admissions across the VCU Health System enterprise.

The Patient Flow Coordinator (RN) is a fast-paced, high-visibility, & high-impact position serving at the heart of the organization as the central hub for intra- & interfacility patient transfers & bed placement operations. This role collaborates with clinical leaders in the inpatient, perioperative, & emergency department settings to coordinate & optimize the safe & efficient movement of patients from all points of entry throughout the health system. A high level of clinical expertise & critical thinking, collaboration, & multitasking are required to effectively triage by patient acuity & clinical needs, evaluate capacity, & optimize throughput in compliance with hospital policies, EMTALA regulations, & departmental procedures to drive optimal patient care outcomes.

Key Responsibilities
• Coordinate patient flow operations for pediatric & adult patient populations across the VCU Health System enterprise
• Triage & assign bed placement based on clinical status, acuity, diagnosis, level of care, nursing competencies, & unit capabilities.
• Act as a clinical & administrative resource for complex patient care & bed assignment situations.
• Facilitate bilateral patient transfers, collaborating with external health systems within the region & beyond, VCU Lifecom, EVS, transport, & clinical team members.
• Identify & address barriers to patient flow in collaboration with physicians, nurse leaders, & operational teams.
• Monitor & guide adherence to established protocols & policies regarding bed placement & patient care delivery.

Required Education:   

Diploma or Associates Degree from a professional nursing program recognized and/or approved by the corresponding state’s Board of Nursing.
All Patient Flow Coordinators without a Bachelor's degree in Nursing (or higher) will be required to enroll in an approved RN to BSN program within two years of their start date and to complete the program within five years of their start date.

Preferred Education:
Master’s Degree in Nursing or related field

Licensure/Certification Required:

Current RN licensure in Virginia or eligible compact

BLS certification

Licensure/Certification Preferred:  N/A

Minimum Qualifications:

Required Qualifications:  Three (3) years clinical experience in an acute care setting

Other Knowledge, Skills and Abilities Required:

Employee must complete 15 contact hours of Continuing Nursing Education during this evaluation cycle. Failure to achieve the required number of contact hours will result in the employee being ineligible to receive an overall performance rating above "Fully Meets".

Five (5) years of clinical experience in an acute care setting

Two (2) years leadership experience to include, but not limited to, precepting, charge nurse experience, leading projects/initiatives

Clinical triage experience

ICU, Operating Room/PACU, or ED experience
