What You’ll Do
Provide comprehensive OT evaluations and treatments for adult and geriatric patients in their homes
Develop individualized care plans focused on functional improvement and safety
Collaborate with interdisciplinary teams including nurses, PTs, and physicians
Educate patients and caregivers on adaptive techniques and equipment
Document care accurately and timely using EMR systems
What You Bring
Current Tennessee OT license (or eligibility)
Minimum of 1 year of clinical experience preferred—home health experience a plus
Strong communication and time management skills
Passion for patient advocacy and holistic care
