Job Description

Serves as a liaison and communicator between all the partners in the clinic, hospital, and community setting involved in the care of the patient. Coordinates prenatal and support service appointments, including language access. Ensures timely and accurate completion of required third party reimbursement applications. Manages resources appropriately. Educates the team about psycho/social factors which impact on the health status of the client. Advocates for individual and community needs. Assist in the collection and tabulation of data for reports and records. Assures compliance with WMC policies and procedures as related to education and outreach activities. Assists in the development of population goals/outcomes. Coordinates scheduling of staff with physician offices and support services Serves as a resource on care management and population specific information.

Education
High School Education or equivalent required with Associates Degree preferred

Certification & Licensures
Trained healthcare interpreter—bilingual Spanish/English required. Valid VA Driver's License.
