About this role

100% FTE, Monday – Friday, 8:00 – 5:00pm. No weekends or holidays. You will be working at Eastpark Medical Center in Madison, WI.

We are seeking a Registered Nurse (RN) Navigator to:

  • Work in collaboration with numerous multidisciplinary teams to help navigate patients and families through Pulmonary care.
  • Be a conduit of information between inpatient and outpatient services and pre- and post-surgical care for patients and families.
  • Participate in continuous quality improvement initiatives.
  • Work closely with Pulmonary providers, carry out coordinated plan of care for patients, and be highly organized and great at multi-tasking.

Qualifications
  • Bachelor's Degree in Nursing (BSN) Required
  • Master's Degree in Nursing, Social Work, Public Health, Healthcare Administration, or related field Preferred
Work Experience
  • 3 years of relevant clinical nursing experience Required
  • 1 year of relevant experience in the specific patient population being supported Preferred
Licenses & Certifications
  • Registered Nurse license in the state of Wisconsin Upon Hire Required
  • Basic Life Support/CPR Upon Hire Required
  • Applicable Oncology certification based on patient population Preferred
