JOB SUMMARY
1. Provides direct nursing care, including patient observation and delivery of prescribed treatments. Conducts initial and ongoing comprehensive assessments of home health patient needs, including OASIS assessments at appropriate time points. 2. Evaluates outcomes of patient care and identifies discharge planning needs. Consults with other disciplines as required. Adjusts nursing care processes as necessary. 3. Maintains patient records and charts to ensure accurate record of treatment, including med profile, care plan, communication notes, etc. 4. Provides instructions to patients/families for use of products and services utilizing nursing process to address learning needs. 5. Prepares, administers, monitors, and discontinues IV fluids. Accesses IV delivery system (ie. peripheral IV start, implant cath, broviac, etc.). 6. Administers medications and treatments as prescribed by the physician.
