The Advanced Practice Registered Nurse (APRN) provides assessment, management and outcome evaluation for patients in inpatient and outpatient settings. The APRN serves as a resource to educate patients, families, professional nurses, and staff; functions as a liaison between patient, family and UofL Physicians staff; and participates in clinical research. The APRN should demonstrate an advanced level of medical and nursing knowledge, clinical and technical competency, sound clinical judgment, professionalism as it relates to interpersonal and general communication skills, timely and compliant documentation, and takes responsibility for ongoing professional development and competency validation.

Acute Care - Advanced Practice Provider Fellowship

Job Summary

This job is a 12- month training position that utilizes an inter-professional approach and professional development to prepare an advanced practice fellow to embrace the independent and fast-paced role of an Advanced Practice Provider. The program will include seven required core rotations and 12 weeks of elective time.

There will be didactics, precepted and autonomous practice and clinical skills preparation. The curriculum will focus on knowledge advancement, professional development, leadership, inter-professional communication, diversity and inclusion, and self-care practice. All fellows will work under the direct supervision an assigned APP preceptor in addition to physician colleagues and program director leadership.

At the end of this program the APP fellow will be able to independently practice using critical thinking skills and care plan development to progress their patients throughout the continuum of the healthcare system. Fellows are expected to identify health disparities and collaborate with other members of the health care team to meet the needs of the patient population we serve.

Essential Functions

· Provide direct patient care to an assigned load of patients determined by the service assigned.

· Perform comprehensive relevant health, social, medical history and performing thorough physical assessments.

· Identify and evaluate health and social disparities influencing a patient’s health and well-being.

· Diagnose and understand the etiology of the chief complaint and be able to develop a plan of care for that patient, while considering cost reduction, health equity, and diversity in those plans of care.

· Identify priorities to satisfy the needs of the patient, family, and community.

· Order and interpret appropriate labs and tests, while reducing waste and promoting maximum benefit to the patient.

· Order and monitor effects and response to medications, therapies, and other interventions.

· Review documentation and being prepared for rounding with the physician team to provide additional details to positively impact patient rounds.

· Consult appropriate services and provide them with a succinct synopsis of the patient and reason for the consultation.

· Ensure thorough patient/family education of identified learning needs considering health literacy, ensure appropriate follow-up arranged, and communicate clearly with patients and families next steps in their care.

· Implement plans of care that are appropriate and inclusive of the patients’ preferences, beliefs, and abilities.

· Function as a patient and family advocate always.

· Evaluate effectiveness of interventions, tests, and medications ordered and modify plan of care as required.

· Complete documentation of care provided in a timely manner in EHR.

· Take care of any administrative and/or clerical duties in a timely manner.

·  Perform in all fellowship planned activities (didactics, lectures, KNOW modules, and any other assigned activity) as scheduled, complete all pre-work prior to starting a rotation, and submitting any assigned work or evaluations on time.

· Maintain at a minimum monthly meeting with the Program Director.

· Must be organized and able to keep track of own responsibilities.

· Develop and maintain professional and effective communication skills.

· Participate in all service-based education offerings while adhering to any dress code or preparation standards provided to you.

· Maintain fellowship journal that will be used for evaluations and regular check-in meetings with program director.
