DETAILED JOB DESCRIPTION
Assess needs and locate community resources for patients and families in need of concrete services such as transportation, lodging and food. Develop/maintain effective working relationships with community agencies.
Assist patients/families with issues requiring intervention within the hospital and community.
Consult and collaborate with health care providers and other staff regarding patient care, problems and discharge planning.
Counsel, plan and implement discharge and post-hospital care with patients and families considering age specific needs.
Document social services provided appropriately and in accordance with department/hospital policy and maintain department statistical records. Participate in department and hospital committees as assigned.
Interview patients and families for social information relevant to treatment and health care planning and compile comprehensive social evaluations.
Provide crisis intervention, emotional support, counsel for adjustment to illness/disability for personal/family problems related to health/illness on bio-psychosocial issues and complex situations.
Refer patients and families to community, state and federal agencies and resources and provide coordinating services.
Perform legal reports for suspected child abuse, elder abuse, domestic violence, and guardianship referrals.
May complete unit/department specific duties and expectations as outlined in department documents.
REQUIRED QUALIFICATIONS
Master of Social Work degree.
PREFERRED QUALIFICATIONS
Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) licensure by the Missouri State Committee for Social Workers board.

PHYSICAL DEMANDS
The physical demands described here are representative of those that must be met with or without reasonable accommodation. This position is very active and may require ambulating, remaining in a stationary position and positioning self to reach and/or move objects above the shoulders and below the knees. The employee may frequently lift and/or move objects up to 10 lbs. and occasionally move objects up to 25 lbs.
This role may also routinely come into contact with patients who may have contagious illnesses. This position is in a health care facility open 24 hours a day, 365 days a year. Rotating shifts as well as holiday and work during inclement weather will be required.
