The Intensive Support Professional position is responsible for the provision of hands-on intensive rehabilitation services to adults with mental health, intellectual disabilities, and/or co-occurring conditions residing in an Intensive Residential Service Home. Key responsibilities include assisting with medication adherence, appointment compliance, comprehension, crisis planning, behavior modification, and skill building. The position focuses on providing person-centered, strength-based, and trauma-informed care to support individuals in stabilizing and reintegrating into the community.

Responsibilities

Provides skill-based training, support, and interventions to clients residing in an Intensive Residential Service Home (IRSH) and assists clients in residing in the most integrated setting possible based on the member’s needs. Services provided are unique to the individual as identified in their person-centered service plan and may include but are not limited to development of daily living skills, symptom management, accessing physical, mental health, and substance use care and treatment, communication, transportation, and social skills.

Participates in client service planning and development of measurable goals based on client needs and desires.

Provides supervision of client self-administration of medication as identified in the person-centered service plan.

Consults with natural and professional supports and community agencies and resources to assist clients with living in the least restrictive setting.

Advocates for client rights and choice in obtaining needed services.

Attends regularly scheduled staff meetings, debriefings, rounding, and in-services.

Participates in and completes all required training by identified due date.

Participates in the planning, implementation, and evaluation of programming effectiveness.

Completes housekeeping and home maintenance tasks as identified in the daily, weekly, and monthly checklists.

Assists individuals in the development of natural supports within the community.

Engages in the creation and execution of crisis plans as necessary.

Recognizes early symptoms of physical and mental illness, and performs appropriate interventions, communication, and/or referrals in accordance with the identified crisis plan.

Provides support and interventions to assist with crisis de-escalation.

Provide skill training related to crisis intervention techniques and coping skills.

Provides education regarding available supports and accessing appropriate supports based on each unique situation. Assists individuals in using the least restrictive crisis resource possible if external resources are needed.

Participates in staff debriefing and assists the team with identifying effective de-escalation and crisis management skills and techniques based on client stated preferences and needs.

Completes documentation of services provided accurately, thoroughly, and timely within established guidelines.

Completes documentation of the supervision of client self-administration of medication as required in the electronic Medication Administration Record.

Ensures documentation meets billing requirements and completes necessary corrections timely as required.

Participates in and/or facilitates HCBS required safety drills, inspections, etc. as assigned.

Minimum Requirements

Education:

 

 

 

High School Diploma

Bachelor’s degree in a human services related field (including psychology, social work, nursing or education)

Experience:

 

 

 

 

One Year Experience in Behavioral or Mental Health

One Year Experience in Behavioral or Mental Health

License(s)/Certification(s):

 

 

 

 

Valid driver’s license when driving any vehicle for work-related reasons.

Successful completion of minimum 30 hours of crisis management and de-escalation training within the first year of service provision

Knowledge/Skills/Abilities:

 

 

 

 

· Understands, follows, and advocates for individual’s rights;

 

· Possess strong de-escalation and crisis management skills, and ability to effectively manage emergency situations

 

 

 

Other:

 

 

 

Use of usual and customary equipment used to perform essential functions of the position.
