Our growing Liver Clinic is seeking an experienced and compassionate Advanced Practice Provider (Nurse Practitioner) to join our multidisciplinary hepatology team. This role provides comprehensive care for patients with a wide range of liver conditions, including cirrhosis, hepatitis, fatty liver disease, autoimmune liver disorders, pre- and post-transplant care, and other complex hepatology needs. The APP will work collaboratively with hepatologists, nursing, and support staff to deliver high-quality, patient-centered care. With the goal after training to see 12-15 patients per day, pending complexity.

Responsibilities

  • Monday-Friday 8-4:30
  • Conduct new and follow-up patient visits, including assessments, diagnosis, treatment planning, and ongoing management of liver-related conditions.
  • Coordinate care for high-acuity and chronically ill patients, ensuring seamless communication across the care team.
  • Order and interpret diagnostic tests, labs, imaging, and specialty evaluations.
  • Manage medication adjustments, including hepatology-specific therapies and monitoring for side effects.
  • Provide patient and family education related to liver disease, treatment plans, lifestyle interventions, and long-term care needs.
  • Participate in care pathways for cirrhosis management, transplant evaluations, and post-transplant follow-up as applicable.
  • Collaborate with inpatient teams to support transitions of care and reduce readmissions.
  • Document all patient interactions in the electronic medical record accurately and in a timely manner.
  • Contribute to quality improvement initiatives, care standardization, and process development within the clinic.
  • Outreach clinic at least once monthly and could involve more pending volumes.

Qualifications

  • Licensed Nurse Practitioner (NP) in the state of Iowa
  • Minimum of 2 years of experience in GI/hepatology, internal medicine, or a related specialty preferred.
  • Demonstrated ability to manage complex patients in an outpatient specialty setting.
  • Strong communication, critical-thinking, and patient education skills.
  • Ability to work effectively in a multidisciplinary team.
  • EMR proficiency (Epic preferred).
