About this role

$750 Sign on bonus

New employment opportunity with Northeast Career Planning at Unity House of Troy for a Student to Work Counselor. 

Key role in assisting youth with disabilities or mental health challenges with transitioning from high school to work or onto furthering their education. 

Responsibilities:

Design and implement vocational and transitional programs aligned with the IPS model. 

Participate in IEP and ITP meetings to develop individualized transition plans. 

Collaborate with educators, mental health professionals, and families to integrate vocational goals with therapeutic support. 

Focus on competitive employment through vocational and mental health services available. 

Develop individual job searches and job development. 

Develop relationships with local businesses and training institutions. 

Conduct employer outreach to identify job opportunities tailored to student/youth’s interests. 

Educate employers about accommodations and supports available through the IPS model. 

Assist in exploring mainstream education and vocational training programs. 

Advise on financial aid applications (e.g., FAFSA), disability services, and academic planning for those continuing their education. 

Provide coaching on study skills, time management, and navigating campus resources

Requirements:

Bachelor’s degree in related field preferred. Experience with IPS, supported employment and or working with youths with disabilities or mental health conditions referred. Valid and clean NYS driver license. Must be able to travel locally. Strong interpersonal and communication skills. Must be able to work collaboratively in a multidisciplinary team as well as independently. Must be able to arrive on time and complete all reports on time.
