Associate's degree salary starts at: $65,298.87

Unity House is dedicated to improving the lives of individuals in our community, and our Clinical Services Department is seeking a compassionate and skilled Mental Health RN to join our CTI Team.

CTI is a time-limited, evidence-based care management model designed to ensure continuity of care for individuals with severe mental illness during transitions from inpatient settings back into the community. This position offers the opportunity to work within an innovative, multidisciplinary partnership between Unity House of Troy (Rensselaer County) and Mohawk Opportunities (Schenectady County).

The Mental Health RN will support participants through integrated care management, health assessments, and wellness planning within their nursing scope of practice.


Key Responsibilities

  • Provide integrated health care and care management services to individuals across Rensselaer and Schenectady Counties.

  • Work as part of a mobile, multidisciplinary care management team.

  • Conduct basic health assessments and identify unmet medical needs.

  • Assist with person-centered care planning.

  • Provide referrals to community medical providers based on client needs.

  • Deliver treatment, rehabilitation, and supportive services.

  • Build and maintain strong relationships with community partners.

  • Complete required documentation and administrative tasks accurately and on time.

  • Promote wellness, self-management, and holistic care.

  • Foster a culture of inclusion and belonging.


Qualifications:

  • Current, unrestricted NYS Registered Nurse (RN) License.

  • Minimum 1 year of experience working in mental health and/or substance use settings.

  • Valid NYS driver’s license with reliable vehicle

  • Ability to work both independently and collaboratively.

  • Proficiency with computers and electronic documentation.
