General Summary:

Under the direction of the Manager – Diabetes Management Services, the Diabetes Educator (Certified) is responsible for providing education on diabetes self-management to newly diagnosed or poorly controlled diabetic patients. Collaborates with care team to coordinate, identify clinical problems, and close gaps in care. Will assist the manager to plan, develop, promote and evaluate the program on an ongoing basis.

Requirements:

  • Registered Dietitian or Registered Nurse in the State of Michigan.
  • Bachelor of Science in Nursing preferred
  • Current basic life support (BLS/CPR) preferred (required upon hire).
  • Certified Diabetes Care and Education Specialist (CDCES).
  • Insulin pump certification preferred
  • License and certification(s) must be kept current and applicable.
  • Two years’ experience working with diabetes patients.
  • Proficiency with utilizing computers including Microsoft Office Suite programs, EMR systems, and other required applications.
  • Excellent communication and interpersonal skills.
  • Strong team skills.
  • Ability to work independently.
  • Excellent time management and organizational skills.
  • Knowledge of health promotion and maintenance.
  • Excellent problem solving and critical thinking skills.
  • Ability to establish and maintain a high level of credibility with the physicians, executive management and business stakeholders and clearly articulate complex messages
  • Demonstrated ability to extract pertinent information and reach sound conclusions from a variety of data sources

Essential Functions and Responsibilities:

  1. Conducts assessment to determine patient’s biopsychosocial needs.
  2. Prepares care plan with short and long term goals.
  3. Assesses the outcome of care plan interventions and revises as appropriate.
  4. Provides counseling and education concerning management of diabetes.
  5. Assesses patient’s readiness to learn and perceived needs.
  6. Uses evidence-based teaching methods, behavioral and motivational strategies to educate patients on diabetes management.
  7. Creates an educational setting conducive to learning with adequate resources to facilitate the learning process.
  8. Effectively applies evidence-based research to enhance practice.
  9. Possesses comprehensive knowledge of community resources to serve as referral source to patients.
  10. Documents education, care and service in electronic medical record (EMR).
  11. Adheres to the standards of the American Association of Diabetes Educators Michigan Department of Community Health.
  12. Ensures program statistics and billing are done accurately and in a timely manner.
  13. Participates in quality improvement activities to improve patient care and service.
  14. Performs other duties as assigned. These may include but are not limited to: Maintaining a current knowledge base of department processes, protocols and procedures, pursuing self-directed learning and continuing education opportunities, and participating on committees, task forces, and work groups as determined by management.
