Manage end to end patient care to ensure patients achieve their health goals.</p> <p>2. Consult new patients on the platform to educate them about the Twin program, health conditions, health goals &amp; review medical reports &amp; prescriptions.</p> <p>3. Supervise &amp; encourage patients to achieve their health goals (example reversal of diabetes).</p> <p>4. Encourage patients to follow the program guidelines &amp; adhere to their health &amp; nutritional requirements.</p> <p>5. Retain patients on the program by hand holding them &amp; encouraging to move towards their health goals.</p> <p>6. Own patient outcome for your assigned set of patients.</p> <p>7. Communicate effectively with partner doctors and patients.</p> <p>8. Manage all patients under the guidance of partner doctors &amp; senior twin doctors.</p> <p>9. Manage patients in the care of senior doctors &amp; partner doctors to deliver health outcomes.</p> <p>10. Manage all patient escalations round the clock to ensure highest level of patient safety.</p> <p>11. Follow all Twin protocols &amp; guidelines.</p> <p>12. Gain solid knowledge of company product &amp; metabolic disease reversal.</p> <p>13. Collaborate with internal team (coaches, operations, doctors) to achieve results.</p> <p>14. Be available in all shifts (morning/evening/night) as per requirement.</p> <p><strong>Your Qualifications and Attributes:</strong></p> <p>1. Strong knowledge of diabetes &amp; general patient care.</p> <p>2. Strong knowledge of medical emergencies.</p> <p>3. Experience in online consultations.</p> <p>4. Experience in individual handling of patients.</p> <p>5. Team player</p> <p>6. Good communication skills</p> <p>7. Enthusiastic</p> <p>8. MBBS, Diploma in diabetes/certificate course in diabetes.</p>
