What We’re Looking For

 

Trillium Health Resources has a career opening for a Transition of Care (TOC) Coordinator to join our team! The TOC Coordinator is responsible for managing complex Transition of Care (TOC) functions across the organization and other external agencies.  This position is responsible for administratively supporting the comprehensive clinical review service requests process for persons with Behavioral/Mental Health, Intellectual Developmental Disabilities, and/or Substance Use (BH/MH/IDD/SU) disorders transitioning between plans, working collaboratively with Clinicians to ensure the timeliness of reviews and monitoring outcomes. 

 

On a typical day, you might:

 

  • Manage the TOC Request to Move and Warm Hand Off processes across Trillium and other external agencies.
  • Serve as the primary contact for providers, members, and internal departments regarding Benefit Plan Management and TOC inquiries. 
  • Oversee the timeliness of clinical reviews and track notifications for members or enrollees.
  • Work collaboratively with Clinicians to coordinate the submission of clinical documentation to Peer Reviewers for decisions.
  • Track Peer Reviewer decisions and communicate outcomes to Clinicians.

Qualifications

 

Required:

  • High School Diploma/GED and six (6) years’ experience working in the BH/MH/SU/IDD field; OR
  • Associate’s degree in Human Services, Health, or related fields and four (4) years’ experience working in the BH/MH/SU/IDD field; OR
  • Bachelor’s degree in Human Services, Health, or related fields and two (2) years’ experience working in the BH/MH/SU/IDD field; OR
  • Equivalent combination of education/experience.
  • Must have a valid driver’s license.
  • Must reside within North Carolina.
  • Must be able to travel within catchment as required.

 

 

Preferred:

  • Experience working with the BH/MH/SU/IDD population.
