What We’re Looking For

 

Trillium Health Resources has a career opening for a TCL Transitions Complex Care Unit Manager to join our team! The TCL Transitions Complex Care Unit Manager is responsible for overseeing staff in the implementation of evidence based industry standard practices to support members. The TCL Transitions Complex Care Unit Manager provides oversight to ensure members receive whole person integrated care, to facilitate positive outcomes for members. This includes provision of input for complex care coordination and provision of oversight to staff to ensure members receive appropriate assessment, discharge/transition planning, and linkage to services in the community. 

 

On a typical day, you might:

 

  • Provide complex care coordination to assigned individuals who may have identified needs with behavioral/mental health, physical health, co-occurring, co-morbid, or multi-morbid conditions.
  • Collaborate with internal staff across disciplines/teams (Care Coordinators, Clinicians, OT, COTA, Housing Specialists, Peers, etc.) to facilitate integrated care.
  • Monitor the Care Plan (physical, behavioral health, and social determinant concerns), service delivery, and health and safety of the member.
  • Perform clinical functions of discharge/transition planning and diversion, including clinical interviewing; obtaining and reviewing clinical records; identifying potential treatment needs; assessing barriers to treatment and recommending solutions; and assessing general health needs and recommending referrals.
  • Oversee collaboration between internal and external staff to coordinate discharge planning from Community Hospitals for primary medical admission.

Qualifications

 

Required:

  • Bachelor’s degree with a minimum of one (1) year of documented experience working with the co-occurring of physical health and behavioral/mental health, physical health, or chronic condition population; OR
  • Equivalent combination of higher education/experience.
  • Fully licensed by the North Carolina Board of Nursing as a Registered Nurse (RN).
  • Must have a valid driver’s license.
  • Must reside within Trillium’s Catchment area, which includes the following counties: Anson, Bertie, Beaufort, Bladen, Brunswick, Camden, Chowan, Carteret, Columbus, Craven, Currituck, Dare, Duplin, Edgecombe, Gates, Greene, Guilford, Halifax, Hertford, Hoke, Hyde, Jones, Lee, Lenoir, Martin, Montgomery, Moore, Nash, New Hanover, Northampton, Onslow, Pamlico, Pasquotank, Pender, Perquimans, Pitt, Randolph, Richmond, Robeson, Sampson, Scotland, Tyrrell, Warren, Washington, Wayne, or Wilson. 
  • Must be able to travel within catchment as required.

 

Preferred: 

  • Advanced degree 
  • Experience in all three (3) areas listed above.
  • Supervision of two (2) or more employees.
