What We’re Looking For
Trillium Health Resources has a career opening for a My Choice Transition Care Manager to join our team! This position's primary responsibilities include providing care coordination/care management (treatment planning, assessment, monitoring, educating, and referral/linking) to individuals identified in the My Choice - Living Transitions program. This includes addressing physical, behavioral health, and social determinant needs. My Choice Transition Care Manager will provide care planning with foundations in national evidence-based and informed standards to do whole person care, as well as discharge and transition planning from Hospitals, inpatient units, ICF/IID not operated by the state, and state operated Developmental centers.
On a typical day, you might:
- Provide integrated care coordination/care management to members who have identified needs with behavioral health, physical health, co-occurring, co-morbid, and/or multi-morbid conditions.
- Facilitate referrals for health-related services, community-based resources, and social support services.
- Assess members’ needs before and after discharge.
- Ensure completion of the Care Management Comprehensive Assessment (CMCA) and other assessments as needed.
- Ensure timely and accurate documentation of all transition planning/post transition planning and coordination efforts in Trillium Health Resources software platform.

Qualifications
Required:
- Must meet QP status.
- Fully licensed by the NC governing board regulating Human Services professions; OR
- Fully licensed RN who is licensed to practice in the state of NC by the NC Board of Nursing who also has four (4) years of full-time experience with the MH/DD/SA population served; OR
- A graduate of a college or university with a Master’s degree in a human service field and has one year of full-time, pre or post-graduate degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has one year of full-time, pre or postgraduate degree accumulated supervised experience in alcoholism and drug abuse counseling; OR
- A graduate of a college or university with a bachelor's degree in a human service field and has two years of full-time, pre or post-bachelor's degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has two years of full-time, pre or post bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling; OR
- A graduate of a college or university with a bachelor's degree in a field other than human services and has four years of full-time, pre or post-bachelor's degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has four years of full-time, pre or post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling.
- Additionally, if serving members with LTSS needs, the care manager must meet the minimum QP requirements defined above (A-D) and shall additionally have at a minimum two (2) years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience. This experience may be concurrent with the years of experience required to become a Qualified Professional.
- The following immunizations are required for this position:
- Fully vaccinated against Covid-19, Influenza (annually within Influenza season Nov 1 – March 31), MMR, Varicella and Tdap (Tetanus, diphtheria pertussis).
- MMR and Varicella - must show proof of vaccination or have blood drawn to show immunity.
- Tdap - must show proof of one dose.
- Employees must provide proof of all immunizations prior to their start date or agree to be and provide proof of all immunizations within 4 months of start date.
- Must have a valid driver’s license.
- Must reside within Trillium’s catchment area of North Carolina, which includes the following counties: Anson, Bertie, Beaufort, Bladen, Brunswick, Camden, Chowan, Carteret, Columbus, Craven, Currituck, Dare, Duplin, Edgecombe, Gates, Greene, Guilford, Halifax, Hertford, Hoke, Hyde, Jones, Lee, Lenoir, Martin, Montgomery, Moore, Nash, New Hanover, Northampton, Onslow, Pamlico, Pasquotank, Pender, Perquimans, Pitt, Randolph, Richmond, Robeson, Sampson, Scotland, Tyrrell, Warren, Washington, Wayne, Wilson.
- Must be able to travel within catchment as required.
Preferred:
- License and/or certification may be accepted as noted above.
- Preference will be given to individuals with experience transitioning members with IDD into community living arrangements.
