About the Local Delivery Group: 

The Local Delivery Group (LDG) is a collaborative network of hospitals and healthcare partners within a defined region, established to drive the implementation and optimization of digital health solutions that improve patient care, streamline workflows, and enhance system integration. LDG Digital refers to the suite of digital programs, platforms, and initiatives managed by the LDG, focusing on technologies such as eReferral and Central Intake to facilitate efficient, secure, and patient-centred access to health services across member organizations. 

As the lead organization for the Central South LDG, Trillium Health Partners plays a pivotal role in coordinating regional digital efforts, setting strategic direction, and supporting the deployment and adoption of innovative digital health solutions. THP is responsible for championing best practices, ensuring alignment with provincial and regional priorities, and fostering collaboration among LDG members to advance digital transformation and improve health outcomes throughout the region. 

Position Overview  

The Project Coordinator, LDG role helps support the planning, coordination, and execution of digital initiatives and partnership projects within a fast-paced digital health environment. Reporting to the Project Director, Epic Connect, this role provides day-to-day project support, assists with documentation, tracks progress, and helps coordinate resources and communications among internal teams and partner organizations. 

 

The Project Coordinator plays a key role in supporting project workflows, collecting and organizing project information, assisting in stakeholder engagement, and ensuring timely and accurate reporting. This role is essential to keeping digital initiatives moving smoothly and ensuring project teams are aligned, informed, and supported throughout all phases of delivery. 

 Key Responsibilities:    

  • Assist project resources and leadership in planning, scheduling, and monitoring digital project activities.  
  • Help maintain project plans, status reports, action logs, RAID logs (risks, actions, issues, dependencies), and meeting minutes.  
  • Support preparation of project documentation including charters, requirements, technical specifications, and transition-to-operations materials.  
  • Support tracking of project milestones, deliverables, timelines, and dependencies, escalating concerns as needed. 
  • Coordinate meetings, workshops, stakeholder interviews, and site visits.  
  • Support relationships with partner organizations by responding to routine inquiries and gathering information on needs or issues.  
  • Assist with preparing communication materials such as updates, presentations, dashboards, and reporting summaries. 
  • Help ensure documentation is complete, accurate, and aligned with governance standards.  
  • Support version control, documentation archiving, and consistent use of templates.  
  • Assist with reviewing requirements, statements of work, and business cases for completeness. 
  • Coordinate scheduling, project calendars, and logistics for internal and partner-facing activities.  
  • Maintain organized project libraries and shared drives. 
  • Assist with gathering training materials, readiness checklists, and change management outputs.  
  • Support data collection for benefits realization and lessons learned.  
  • Help coordinate go-live preparation tasks and postimplementation follow-ups.

Qualifications:   

  • University degree or college diploma (nearing completion) in Health Informatics, Business, Computer Science, Project Management, or related field.  
  • 1–3 years of experience supporting projects, operations, or administrative functions (healthcare or IT environment preferred).  
  • Experience with documentation, scheduling, coordination, or stakeholder communication.  
  • Proficiency with Microsoft Office tools, especially Excel, PowerPoint, Word, and basic familiarity with MS Project or other PM tools.  
  • Strong organizational, analytical, and communication skills.  
  • Experience in healthcare, digital health, or a large complex organization is an asset.  
  • Knowledge of project management methodologies (PMBOK, Agile, waterfall, hybrid) considered an asset.  
  • Ability to work in a fast-paced environment with multiple priorities and tight timelines.  
  • ITIL, CAPM, or interest in obtaining project-management certifications is an asset.
