About this role

Job Summary

Responsible for planning, organizing, and optimizing specialized pharmaceutical care to improve patient outcomes in a defined area of clinical expertise. Provides direct patient care services, working collaboratively with interdisciplinary clinical teams to support acute and chronic disease management, medication therapy optimization, and safe transitions of care. Collaborates with pharmacy and unit leadership on staff training, clinical education, and the evaluation of pharmacy services across the continuum of care. Participates in formulary reviews, medication use evaluations, and multidisciplinary initiatives. Acts as a clinical resource, promotes high standards of care, and fosters effective partnerships across departments, with a strong emphasis on patient-centered service.

Education Requirements

  • Completes and maintains all competencies and trainings as required for role.
  • Doctorate Degree or combination of relevant education & experience may be considered in lieu of Doctorate Degree

  • Relevant Experience

Certification and Licensure

  • Pharmacist License (PA Pharmacy License must be obtained within 90 days of hire)
