Be a part of a world-class academic healthcare system, University of Chicago Medicine, in this role as an Intake & Referral Nurse Liaison (RN) in our Home Health department for a Full Time, Days shift. We have expanded our home care services to keep pace with shorter hospital stays, evolving technology, changing insurance reimbursement and a growing elderly population. For more than 20 years, our staff has striven to provide seamless, high-quality care for patients of any condition or stage in life. This position is a 100% onsite opportunity. You will need to be based in the greater Chicagoland area. Please note this position will work out of our Ingalls Memorial Hospital campus primarily.

 

The Home Health Intake & Referral Nurse Liaison (RN) works closely with hospital discharge planners and meets with patients and families to facilitate the home health admission process.

 

Essential Job Responsibilities

  • Completes an initial assessment of patient and family to determine home health care needs.

  • Provides a complete physical assessment and history of current and previous illness(es).

  • Educates the patient and family (caregiver) in ways to achieve home health goals.

  • Provides health care instructions to the patient, family (caregiver) as appropriate per assessment and/or plan.

  • Provides direct patient care as needed.

  • Documents the initial patient and family assessments and interventions.

  • Performs initial communication with the physician regarding the patient’s needs.

  • Ensures that arrangements for equipment and supplies are made.

  • Customer/Referral Source Management: Primarily works with designated hospital and other potential referral sources to develop and promote awareness of home health care services, discuss the agency’s capabilities, and educate on the services provided by home health. Facilitates communication between the referral source and the home health department.

  • Works closely with clinical staff to ensure that necessary services are provided.

  • Collaborates with the Intake Departments, scheduling, and clinical staff to manage referrals/admissions.

  • Provides coverage for the Hyde Park campus on occasion and as needed to ensure Home Health Liaison is available to families and patients 

  • Commits to one hundred percent (100%) patient and customer satisfaction by always exhibiting a courteous and helpful manner during interactions with others, including patients, families, visitors, physicians, students, and co-workers.

  • Other duties as assigned

 

Required Qualifications

  • Graduate of an accredited school of nursing
  • Current state of Illinois nursing license as an RN
  • Current Basic Life Support (BLS) certification through the American Heart Association (AHA)
  • One to two years of recent Home Health Care experience
  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills & nursing skills per competency checklist
  • Possess and maintain a valid driver’s license, current automobile insurance, and has personal & dependable mode(s) of transportation.

 

Preferred Qualifications

 

  • Previous Home Care Liaison work experience

Compensation & Benefits Overview

 

UChicago Medicine is committed to transparency in compensation and benefits.  The pay range provided reflects the anticipated wage or salary reasonably expected to be offered for the position.

 

The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data, reviewed on an annual basis. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations, such as internal equity. Pay ranges for employees subject to Collective Bargaining Agreements are negotiated by the medical center and their respective union.
