About the Role:
As a Registered Nurse/Field Staff Coordinator, you'll play a pivotal role in providing and coordinating patient-centered care for individuals in the home health setting. Responsibilities Include:
- Performing initial and ongoing patient assessments
- Executing physician orders and implementing nursing interventions
- Developing and updating individualized plans of care
- Supervising home health aides and participating in performance improvement initiatives
- Coordinating interdisciplinary care and managing referrals
- Assisting in intake coordination and staff scheduling when needed
- Current RN license from the Alabama Board of Nursing
- Minimum of 3 months of nursing experience (home health experience a plus)
- Strong clinical judgment and patient advocacy skills
- Familiarity with Medicare Coverage Guidelines, OASIS, and Home Health Conditions of Participation
- Excellent communication and organizational abilities
- Adolescents (13–18)
- Adults (19–65)
- Geriatrics (65+)
- Moderate physical activity including walking, standing, kneeling, and lifting
- Exposure to home care environments and occasional blood-borne pathogens
- Must be comfortable navigating various home conditions and travel within the service area
